Uploading documentation

There are two ways that you can upload documentation into your EverCheck Wallet account: 

  1. In the required sections of the "Credentials" portion of your provider application 
  2. In the "Documents" tab

Let’s take a look at each. 

Uploading documents as required for your provider application
To complete certain sections of your application, like Provider health, you’ll be required to upload supporting documentation. Follow these steps to successfully upload your documents:

  1. After logging into your EverCheck Wallet account and navigating to your provider application, you’ll see the screen to the right. Click "Provider health."

    Please note: In this example, the provider has completed the "Provider demographics" section and is moving onto the next section, "Provider health." Your application may look different than the example shown here depending on how much of the application you’ve completed.
  2. Here, you’ll be prompted to enter information and attach documentation. This includes uploading proof of your Influenza vaccine and chest x-ray (if applicable). You may drag and drop the document(s) from your desktop directly onto this page, or select "browse" to upload the document(s) from your files.

    The following file types are accepted: PDF, TXT, PNG, JPEG, JPG, DOCX, and DOC.

  3. Next, you’ll be able to review the file name of the document(s) you uploaded and confirm if they’re correct. If you uploaded the incorrect document, you can remove it by clicking the "X" button to the right of the file name. Once you’ve answered the questions in this section and attached the correct documentation, click "Continue."

Any documents that are submitted as a part of your provider application will also be visible in the "Documents" tab.

You will follow the same process for uploading documents in other sections of your provider application.


Uploading documents directly in the documents tab
You can also store important documents separate from your provider application in the "Documents" tab. Here’s how: 

  1. After signing into your account at evercheckwallet.com, click the “Documents” tab on the dashboard.
     

  2. You may drag and drop the document from your desktop directly onto this page, or select "browse" to upload the document from your files.

    The following file types are accepted: PDF, TXT, PNG, JPEG, JPG, DOCX, and DOC.

  3. Once the document has finished uploading, it will appear in the documents list.

You may view or delete the document by clicking the three dots to the right of the file name.

If you choose to upload case logs in the "Documents" tab, you’ll be required to remove any patient identifiers, including: 

  • Patient name
  • Date of birth 
  • Social security number
  • Patient name
  • Account number or patient number with the hospital affiliation 


Please note: Any documents stored in the "Documents" tab of your account will be visible to your workplace.
